Completing an Online Referral Form
GP Referral Forms and Sleep Study Request Forms are in PDF format and can be completed by typing in the highlighted areas within your web browser. Alternatively the form can be saved to your PC. The saved form can then be used as a template for future referrals, e.g. so you don't have to type in your details every time a referral is created.To view fields that require completion click the "Highlight fields" checkbox in the top right hand corner of the document.
Once the form has been completed online, print the form by clicking on the Print Form tab, sign the form and then faxed it to (08) 8277 6890.
You must have the latest version of Adobe Reader installed on your PC.
